up to accept credit cards, all book orders must be prepaid by
check. Personal checks are accepted. Sorry, we cannot ship outside
the U.S. or accept foreign funds or checks drawn on a non U.S.

2. Fill in quantity,
extend subtotal, add sales tax (if applicable) and shipping. Sorry,
no foreign delivery), and total.
3. Make your check out

4. Mail completed form
